흡연하면 치주질환이 정말로 더 심해지나요?

담배에는 니코틴, 니트로사민, 벤젠, 알데히드, 일산화탄소 등 약 5000 개가 넘는 유해 물질이 포함되어 있다고 알려져 있다(1). 이러한 유해물질로 인해 흡연시 심혈관계 질환 (급성 심근경색, 뇌졸증, 말초 심혈관 질환 등), 폐암, 그리고 만성 폐쇄성 폐질환 등의 위험성이 높아진다 (2) 이뿐만 아니라, 흡연은 구강 질환과도 연관이 있는데, 구강암, 구강 점막 질환, 그리고 치주염의 위험 요소로 작용한다 (3).

치주 질환과 흡연과의 관계는 1940년대 처음 연구되었는데, 핀포그 등은 괴사성 궤양성 치은염이 흡연과 관련이 있다고 하였다 (4). 미국 치주 학회에 따르면 흡연자의 경우 비흡연자에 비해 치주염의 위험이 2.6배 ~ 6배 더 높다고 보고하였다 (5). 또한, 흡연자들의 경우 부착 소실이 더 크고, 치조골 소실도 더 많다고 알려져 있다 (6, 7). 그리고 치석의 유무를 살펴본 연구에 의하면 비흡연자에 비해 흡연자들의 치은 연상 및 치은 연하 치석이 더 많이 나타난다 (8, 9).

흡연시 치주조직의 생리 및 면역에도 다양한 영향을 끼친다. 만성적으로 모세혈관의 흐름이 감소되고, 치은 조직의 기능 이상 및 치주 질환을 일으킬 수 있다 (10, 11). 그리고, 중성구의 운동 (phagocytosis) 을 방해하고, 과산화수소 및 초과산화물과 같은 염증 반응 물질의 분비 이상 등에 영향을 끼친다 (12). 흡연 환자들에게서는 A. Actinomycetemcomitans 라는 치주병원균에 대한 면역글로블린 (Ig G2) 의 분비도 감소하고 (13), IL-4, B 세포가 감소하고, 치은열구액 내 TNF-α and IL-6은 증가한다는 보고가 있다 (14).

치주염 환자들의 치주치료 시 흡연은 그 결과에 영향을 준다. Heasman 등(15) 의 review 논문에서는 흡연자들의 치주 치료 후 치유가 비흡연자들에 비해 더 좋지 않았다고 보고하고 있다. 흡연자들의 경우 스케일링과 잇몸치료를 시행한 이후에도 치주질환과 연관이 있는 세균 (T. forsythia, P. gingivalis, T. denticola) 들이 비흡연자와 비교시 크게 줄어들지 않았다는 보고도 있다 (16).

이와 같이 흡연은 치주질환과 큰 연관성이 있으며, 건강한 치주 상태를 유지하기 위해서는 금연을 하는 것이 바람직하다. 올해 2월 25일부터 국민건강보험공단에서는 금연치료 지원사업을 시행하고 있으며 치과에서도 금연 치료 프로그램을 신청할 수 있다. 12주 간 금연 상담 및 금연치료 의약품을 처방 받을 수 있고, 금연 보조제의 비용을 지원 받을 수 있다. 올해부터 금연을 결심하였다면 가까운 치과 병원에 찾아 도움을 받아 보는 것도 좋을 것이다.
